Коррекционный центр психического здоровья: гибридная модель для лиц с психическими расстройствами

По мере проведения курса на деинституциализацию и сокращение числа стационарных психиатрических коек отмечается повышение распространенности тяжелых психических расстройств в тюрьмах. Эта тенденция сохраняется несмотря на инвестирование в развитие служб охраны психического здоровья в сообществе по месту жительства, служб по работе с зависимостями, судебно-экспертных служб, а также в развитие судов, рассматривающих дела психически больных и наркозависимых подсудимых. Исправительные центры, располагающие ограниченными ресурсами и инфраструктурой для охраны психического здоровья, с трудом справлялись с потоком больных, зачастую полностью полагаясь на помещение правонарушителей в одиночный изолятор, что часто приводило к неоптимальным и даже трагическим результатам. Как правило, у правонарушителей с тяжелыми психическими расстройствами имеются сложные криминогенные наклонности и потребности в обретении психического здоровья. Если не проводить работу с этими потребностями, они могут обусловливать как ухудшение состояния психического здоровья, так и рецидивизм. Вместе с тем немного исправительных учреждений, которые полностью посвящают себя работе с данной популяцией. Гибридная модель исправительного центра и центра психического здоровья представляет собой относительно новый подход к работе с потребностями психически больных правонарушителей. В данной статье предлагается обзор работы,
проводимой в одном подразделении, которое стало первопроходцем при реализации данного подхода в Канаде, в провинции Онтарио; также излагаются доказательства эффективности этого подхода.
