The present review of publications aims to analyze and systematize literature data on treatment adherence among patients with posttraumatic stress disorder (PTSD) and comorbid addiction disorders, included searching articles for the period of 1996–2024 in PubMed, ResearchGate and eLIBRARY databases. The presented data shows that PTSD and substance use disorders are prevalent and recurrent mental conditions with complex etiopathogenesis and shared psychological and neurobiological mechanisms. Patients with PTSD and substance dependency are characterized by: more severe clinical profile, low treatment adherence, frequent substance use relapses, and unfavorable prognosis. Provision of comprehensive psychiatric treatment for PTSD and substance use disorders currently includes practical cognitive behavioral therapy techniques that are used to simultaneously address symptoms of both PTSD and substance use related disorders, in order to ensure the best possible therapeutic effect and improve treatment adherence.
