Clinical typology of mixed mania in bipolar affective disorder

Mixed mania is very common among patients with bipolar disorder. A clinical typology of mixed states has been developed as part of an ongoing effort to investigate mixed bipolar states. Based on statistical analysis of MAThyS scale scores four mixed mania variants were singled out: dysphoric mania, retarded mania, asthenic mania, hypochondrial mania. Each of these is characterized by a specific symptomatological profile and correlates with the degree of severity of manic and depressive symptoms and intensity of emotional reactivity. Premorbid affective temperament was found to have a noticeabe impact on the clinical picture of mixed mania, but the extent of this impact tends to vary for each variant. Furthermore, an association was established between a variant of mixed mania and progression and prognosis of bipolar disorder.
