In this analytical scientific review, pursuing the objective of mastering, optimizing and improving the forensic-psychiatric expert assessment of personality disorders in the course of applying the ICD-11 guidelines, we have carried out the analysis of the evolution of the contemporary understanding of this concept from the inception of the term “psychopathy” up to the ideas of the category of “personality disorders”. We elucidate the general theoretical, methodological and methodical issues, the diagnostic and assessment criteria of forensic-expert approaches which have been in use previously. We outline the prospects for resolving the key forensic-expert issues – those of diagnosis and expert assessment of personality disorders currently found in the offender, making a judgement as to his/her responsibility as a subject, the dynamic changes in social dangerousness over time in the process of administering mandatory medical treatment and monitoring its success.
