Распространенность тяжелых психических расстройств в популяции заключенных, содержащихся в исправительных учреждениях, значительно выше, чем в общей популяции населения. За последние 50–60 лет проведение деинституциализации психиатрических больниц в Северной Америке совпало со значительным ростом численности заключенных на душу населения. Трагедия заключается в том, что в настоящее время в нашей системе исправительных учреждений находится больше людей с тяжелыми психическими расстройствами, чем в психиатрических учреждениях. В настоящей статье рассматриваются данные о взаимосвязи между психическим расстройством и совершением правонарушений, а также криминогенные факторы риска и примеры качественной работы по медико-социальной реабилитации правонарушителей. Особое внимание уделено модели работы психиатра по выявлению риска развития криминального поведения заключенных и изучения их потребностей, а также другим моделям, которые могут быть использованы при медико-социальной реабилитации заключенных.
