Применение личностных опросников для оценки психосоматических соотношений у больных сахарным диабетом 2-го типа

Аннотация

В одномоментном ретроспективном исследовании с целью изучения особенностей личности больных сахарным диабетом 2-го типа с помощью психодиагностических тестов, а также оценки их взаимосвязи с контролем гликемии, психическими и соматическими расстройствами были обследованы 165 пациентов (132 женщины, 33 мужчины, средний возраст 53,5±10,7 года). У 163 из них определялся уровень гликированного гемоглобина (HbAc1), а медицинские карты 71 пациента дополнительно были изучены для выявления осложнений диабета и сопутствующих заболеваний. В результате у 96 (58,2%) пациентов диагностированы тревожно-депрессивные расстройства. У 52 (73,2%) пациентов отмечались ишемическая болезнь сердца, инфаркт миокарда и инсульт в анамнезе, ретинопатия, макулярный отек, хроническая болезнь почек, полинейропатии. Обнаружена значимая ассоциация ряда параметров личности с изученными клиническими характеристиками диабета, что важно учитывать при психотерапевтическом сопровождении таких пациентов. Однако невысокая диагностическая специфичность выделенных параметров указывает на то, что применявшиеся опросники позволяют выявить только часть личностных факторов, связанных с клиникой сахарного диабета 2-го типа.
