В статье описываются современные подходы к изучению этиопатогенеза аффективных расстройств, характеризующихся генетической и фенотипической неоднородностью, на основе выявления диагностических и прогностических биомаркеров и эндофенотипов. Приводятся данные зарубежной литературы о нейропсихологических и когнитивных кандидатах-эндофенотипах, выявляемых в семейных и близнецовых наблюдениях с привлечением молекулярно-генетических исследований. Анализируется обоснованность использования в качестве эндофенотипов отдельных нейрокогнитивых показателей у пациентов с аффективными расстройствами, обнаруживающихся как у больных, так и у их здоровых родственников с усилением выраженности пропорционально повышению генетического родства и тесно связанных с полиморфизмом генов.
