In this scientific review, pursuing the objective of summarizing the contemporary data pertaining to the systematics of psychotic disorders in epilepsy, approaches to diagnosing and treating them, we submit the topical problems of schizophrenia-like disorders of epileptic nature. We point out the high rate of their occurrence, diagnostic challenges, and the approaches to pharmacological corrective interventions. We submit the data, pertaining to the challenges faced when diagnosing the first-time psychotic episode if the link to the epileptic paroxysm is not sufficiently obvious. We point out the significant differences in approach taken by various authors towards the classification of schizophrenia-like psychotic episodes, which makes a significant impact both on the approaches to treatment of these conditions, and on the prognostic judgements.
